摘要:
目的 观 察 应 用 疏 肝 健 脾 １ 号 方 联 合 改 良 ＴＳＴ 术 （ ｔｉｓｓｕｅ － ｓｅｌｅｃｔｉｎｇ ｔｈｅｒａｐｙ ｓｔａｐｌｅｒ， ＴＳＴ 术 ） 治 疗 出 口梗 阻 型 便 秘 的 临 床 疗 效 观 察 。 方法 将 ７２ 例 出 口 梗 阻 型 便 秘 患 者 随 机 分 为 ２ 组 ， 治 疗 组 ３６ 例 在 改 良 ＴＳＴ 术 治疗 的 基 础 上 运 用 疏 肝 健 脾 １ 号 方 口 服 ， 对 照 组 ３６ 例 在 改 良 ＴＳＴ 术 治 疗 的 基 础 上 给 予 安 慰 剂 （ 焦 红 糖 水 ） 口 服 。观 察 ２ 组 治 疗 前 后 Ｌｏｎｇｏ 便 秘 评 分 、 初 始 感 觉 阈 值 、 初 始 排 便 阈 值 变 化 ， 比 较 ２ 组 临 床 疗 效 。 结果 治 疗 组 治 疗 后Ｌｏｎｇｏ 便 秘 评 分 、 初 始 感 觉 阈 值 、 初 始 排 便 阈 值 均 降 低 ， 优 于 对 照 组 （ Ｐ ＜０． ０５） ； 治 疗 后 治 疗 组 直 肠 压 力 增 加 ， 肛管 压 力 下 降 ， 直 肠 肛 管 压 力 差 增 加 ， 优 于 对 照 组 （ Ｐ ＜０． ０５） ； 治 疗 组 治 愈 率 ９１． ６７％， 对 照 组 治 愈 率 ８３． ３３％，２ 组比 较 差 异 有 统 计 学 意 义 （ Ｐ ＜０． ０５） 。 结论 疏 肝 健 脾 １ 号 方 口 服 联 合 改 良 ＴＳＴ 术 治 疗 出 口 梗 阻 型 便 秘 的 疗 效 优于 单 纯 应 用 手 术 治 疗 出 口 梗 阻 性 便 秘 。
Abstract:
Objective To observe the clinical effect of Shugan Jianpi No. 1 Decoction combined with modified TST ( tissue - selection therapy stapler) in the treatment of outlet obstructive constipation. Methods 72 patients with outlet obstructive constipation were randomly divided into two groups. 36 patients in the treatment group were treated with Shugan Jianpi Formula 1 on the basis of modified TST. 36 patients in the control group were given placebo ( caramel water) orally on the basis of modified TST. Longo constipation score, initial sensory threshold and initial defecation threshold were observed before and after treatment, and the clinical efficacy of the two groups was compared. Results After treatment, Longo constipation score, initial sensory threshold and initial defecation threshold in the treatment group were lower than those in the control group ( P < 0. 05) ; rectal pressure increased, anal pressure decreased, and anorectal pressure difference increased in the treatment group, which was better than that in the control group ( P < 0. 05) ; the cure rate of the treatment group was 91. 67%, and the cure rate of the control group was 83. 33%. There was significant difference between the two groups ( P < 0. 05) . Conclusion The curative effect of Shugan Jianpi No. 1 Decoction combined with modifiedTST in the treatment of outlet obstructive constipation is better than that of operation alone in the treatment of outlet obstructive constipation.
