摘 要:目的 通过基于16S的二代高通量微生物测序对就诊于医院和社康的痰湿证眩晕患者肠道菌群进行量质及功能代谢差异分析,以期为阐明眩晕发病机制及完善治疗提供一定的依据。方法 收集于深圳市宝安区中医院上合社区健康服务中心和深圳市宝安区人民医院中医科就诊的痰湿证候眩晕患者的大便标本(治疗组,20例),同时搜集同期对照的健康人群的大便标本(对照组,17例)。采用二代高通量16S rDNA微生物测序对粪便中的微生物组进行检测和分析。结果 治疗组肠道菌群的丰度和均匀度较低于对照组样本,且丰度发生降低的菌群主要集中在Bacteroidetes(拟杆菌门)、Bacteroidia(拟杆菌纲)、Bacteroidales(拟杆菌目)、Bacteroidaceae(拟杆菌科)、Prevotellaceae(普雷沃氏菌科)、Bacteroides(拟杆菌属)、Prevotella(普氏菌属)、Megamonas(巨单胞菌属),而治疗组中的Proteobateria(变形菌门)、Gammaproteobacteria(变形菌纲)、Actinobacteria(放线菌纲)、Enterobacteriales(肠杆菌目)、Fusobacteriales(梭杆菌目)、Bifidobacteriales(双歧杆菌目)、Ruminococcaceae(疣微菌科)、Enterobacteriaceae(肠杆菌科)、Lachnospira(毛螺菌属)丰度相对对照组高,此外,研究组患者粪便中出现了丰度显著增高的其他类菌属。研究组的β多样性要低于对照组。治疗组患者肠道菌群中的半胱氨酸和蛋氨酸代谢、甘氨酸、丝氨酸和苏氨酸的代谢、原核生物的碳固定途径以及核黄素代谢的比例要明显低于对照组(P<0.05)。但是治疗组中的氯烷烃和氯烷烃降解、甘油脂质代谢以及其他未分类功能群的比例要明显高于对照组(P<0.05)。结论 痰湿证眩晕患者肠道菌群数量、结构、生物多样性及其代谢功能均发生了显著改变,这可能是痰湿证发生和中药干预的内在微观物质基础。
Abstract:
Abstract: Objective To analyze the differences in quantity,quality,and functional metabolism of gut microbiota in patients with phlegm dampness syndrome vertigo who seek medical treatment in hospitals and social health through secondgeneration highthroughput microbial sequencing based on 16S,in order to provide a certain basis for elucidating the pathogenesis of vertigo and improving treatment. Methods Fecal specimens were collected from 20 patients with phlegm dampness syndrome dizziness who received treatment at the Shanghe Community Health Service Center of Baoan District Traditional Chinese Medicine Hospital and the Traditional Chinese Medicine Department of Baoan District Peoples Hospital in Shenzhen (treatment group,20 cases). At the same time,fecal specimens were collected from healthy individuals who received control during the same period (control group,17 cases). Using secondgeneration highthroughput 16S rDNA microbial sequencing to detect and analyze the microbiome in feces. Results The abundance and evenness of the gut microbiota in the treatment group were lower than those in the control group,and the decreased abundance of the microbiota was mainly concentrated in Bacteroidetes,Bacteroidia,Bacteroidales,Bacteroidaceae,Prevotellaceae,Bacteroides,Prevotella,Megamonas,while the abundance of Proteobateria,Gammaproteobacteria,Actinobacteria,Enterobacteriales,Fusobacteriales,Bifidobacteriales,Ruminococcaceae,Enterobacteriaceae,Lachnospira of treatment group were relatively high compared to the control group.In addition,other bacterial genera with significantly increased abundance were found in the feces of patients in the study group. The β diversity of the research group is lower than that of the control group. The proportion of cysteine and methionine metabolism,glycine,serine and threonine metabolism,prokaryotic carbon fixation pathway,and riboflavin metabolism in the gut microbiota of patients in the treatment group were significantly lower than those in the control group (P<0.05). However,the proportion of chloroalkanes and their degradation,glycerol lipid metabolism,and other unclassified functional groups in the treatment group was significantly higher than that in the control group (P<0.05).Conclusion The number,structure,biodiversity,and metabolic function of gut microbiota in patients with phlegm dampness syndrome vertigo have undergone significant changes,which may be the intrinsic micro material basis for the occurrence of phlegm dampness syndrome and traditional Chinese medicine intervention.
