Abstract:: Objective To provide neuroimaging evidence for the effectiveness of olfactory three - needle therapy for vascular dementia and to explore the central mechanism of olfactory three - needle therapy for vascular dementia by comparing the changes of signal intensity in brain area before and after olfactory three - needle therapy based on resting – state functional magnetic resonance imaging scanning technology. Methods Sixteen patients with vascular dementia were treated with olfactory three - needle therapy. All the patients were scanned with functional magnetic resonance imaging for thefirst time. After 8 weeks of continuous acupuncture and moxibustion treatment, the second functional magnetic resonance scanning was performed and the data were used to analyze the difference of functional magnetic resonance before and after olfactory three - needle therapy for vascular dementia. Results The statistical results of the demographic variables showed no statistical difference in gender, age, education level, height and weight ( p < 0. 05) . Compared before and after treatment, the brain areas with increased ReHo in vascular dementia patients were left dorsolateral prefrontal/ orbital frontal lobe, left ventromedial prefrontal lobe, left Broca area, left auxiliary motor area, right auxiliary motor area, right hippocampus, left hippocampal/ parahippocampal gyrus, left cingulate gyrus/ thalamus, left superior temporal gyrus. The brain regions with decreased ReHo include right middle temporal gyrus/ fusiform gyrus and right angular gyrus/ superior marginal gyrus. Conclusion The neurological targets of olfactory three - needle therapy are mainly located in central executive network, default network, frontal - parietal cortex loop and Papez nerve loop. Its main function is to improve the memory and executive function of patients. Therefore, olfactory three - needle therapy is preferred for vascular dementia patients with memory impairment and executive dysfunction.
